- Package #1: Preventive Dental Plan
- $22 per month. Plan will pay up to $500 towards preventive dental services.
- 2 exams & cleanings per year.
- Package #2: Dental & Vision
- $33 per month, $1,000 per year dental benefits.
- 2 exams & cleanings, 50% coverage for major services such as root canal, surgical extractions, etc.
- Vision: $150 reimbursement for frames, lenses or contacts per year.
- Package #3: Dental & Vision
- $56 per month. Dental is $2,000 per year coverage and also includes dentures and implants.
- Vision includes $200 per year for frames, lenses, contacts.
Who is Eligible … This Medicare Advantage plan is available to anyone who currently is entitled to Medicare Part A and enrolled in Medicare Part B plans. Individuals with ESRD please call. Under certain situations will be able to enroll in the Anthem plan.
When you see “Out-of-Network” providers, you will pay more than if the doctors/hospitals are contracted network providers.
Open Enrollment … The Annual Open Enrollment is from October 15th through December 7th. To enroll simply complete the application and either fax, email or mail the forms back to me. I’ll do the rest to make sure the process is quick and accurate for your convenience. All changes and new enrollments during this open-enrollment will have an effective date of January 1.
